Step 1: Download Required Forms

2. Employment References Form   Download

Step 2: Prepare Your Resume

• Submit a recent resume in PDF format.
• Include detailed work history and job descriptions for the past 5 years.

Step 3: Submit Your Application Packet

• Email the 2 completed forms + resume to: [email protected]
• Email Requirements:
• Send from the same email address listed on your application forms.
• Subject Line: “CTD New Hire – [Your Full Name]”
• Attachments: ALL documents (including resume) must be in PDF format. 

 What Happens Next?

• Confirmation: Caregivers That Deliver will email a receipt confirmation within 48–72 hours of receiving your application packet.
• Phase 2: If your application meets initial criteria, you will be sent a DocuSign invite to begin the background check and pre-onboard forms.
• Phase 3: If you complete both Phase 1 and Phase 2, you will be invited to participate in the final phase of the interview process.

Our Caregiver Hourly Rate Range is: $23 per hour up to $28 per hour [Depending On Experience and Certification]

*** CEU’s are not required at this time. We will notify our current workers when CEU courses and applicable internal training will become available for essential job requirements.
